Lakeview Chiropractic & Acupuncture Wellness Centre is committed to protecting the privacy and confidentiality of your personal information and personal health information. We collect, use, disclose, retain, and protect your information to provide health care and related services, to manage our clinic, to meet professional and legal obligations, and for other purposes permitted by law.

We may collect information directly from you, from other health care providers, from insurers or payers, from referral sources, and from individuals authorized to act on your behalf. We generally use and disclose your information with your consent or as otherwise permitted or required by law.

Our public statement describes our information practices, how to contact our privacy contact person, how to request access to or correction of a record, and how to make a complaint to the clinic and to the Information and Privacy Commissioner of Ontario.
