What Is Sciatica?
Sciatica is a symptom pattern caused by irritation or compression of nerve roots that contribute to the sciatic nerve. Those nerve roots begin in the lower spine, join in the pelvis, and form a large nerve that travels through the buttock and down the leg.
Pain often follows a line from the lower back or buttock into the thigh, calf, or foot. The exact route depends on which nerve root is affected. Some patients have little back pain but significant leg pain, while others experience a combination of back discomfort, tingling, numbness, or weakness.
Sciatica is not a complete diagnosis by itself. A herniated disc, spinal stenosis, arthritic change, or another source may be responsible. Symptoms can begin suddenly after lifting or twisting, or develop gradually with prolonged sitting and reduced spinal tolerance. The examination is used to identify the likely cause and screen for conditions that need medical referral.
Common Symptoms Of Sciatica
Sciatica commonly causes sharp, burning, aching, or electric pain that begins in the lower back or buttock and travels into one leg. The pain may extend behind the thigh, along the outside of the leg, or into the calf and foot.
Tingling, pins-and-needles, numbness, or altered sensation can occur in the same pathway. When a nerve root is more affected, the leg or foot may feel weak, heavy, or difficult to control during walking, stair use, or lifting the front of the foot.
Symptoms may worsen with sitting, bending, coughing, sneezing, driving, or certain lifting positions. Other patients notice more discomfort with standing or walking, particularly when spinal narrowing is involved. Changing position or lying down may provide temporary relief.
The functional impact can include disrupted sleep, limited walking tolerance, difficulty working at a desk, reduced exercise, and fear of normal movement. Progressive weakness, bowel or bladder changes, or numbness around the groin requires urgent medical assessment.
What Causes Sciatica?
A lumbar disc herniation is a common cause of sciatica because displaced disc material or local inflammation can irritate a nearby nerve root. A disc bulge can also contribute when it narrows available space, although many disc changes found on imaging do not cause symptoms.
Age-related joint and bone changes may narrow the spinal canal or the openings where nerves leave the spine. This is often called spinal stenosis or foraminal narrowing and may produce leg symptoms during standing or walking.
Spondylolisthesis, in which one vertebra shifts relative to another, can affect nerve space. Trauma, repetitive loading, prolonged sitting, and reduced trunk or hip capacity may also influence symptom onset or recurrence without being the sole cause.
Less common causes include tumours, infection, fracture, or conditions outside the spine. The diagnosis should be based on the clinical pattern rather than assuming that all pain travelling into the leg is sciatica.
Conditions That Can Mimic Sciatica
Hip arthritis, sacroiliac joint pain, hamstring injury, piriformis-related irritation, peripheral neuropathy, and nerve entrapment farther down the leg can resemble sciatica. Vascular problems may also cause calf pain, heaviness, colour change, or reduced walking tolerance. Shingles can produce burning pain before a rash appears. A detailed neurological and musculoskeletal examination helps distinguish nerve-root symptoms from local joint, soft-tissue, circulation, or systemic causes.
When To Seek Urgent Care
Seek emergency care for new loss of bowel or bladder control, difficulty urinating, numbness around the inner thighs or groin, or rapidly worsening weakness in one or both legs. Prompt medical assessment is also required after major trauma, for sudden severe symptoms with fever, or when pain is accompanied by unexplained weight loss, significant illness, or progressive loss of function.

How We Diagnose Sciatica
Sciatica assessment begins with the location and behaviour of symptoms, including whether pain travels below the knee, what positions aggravate it, and whether numbness, weakness, or bowel and bladder changes are present. Dr. Drazic reviews injury history, work demands, prior episodes, medical conditions, and any existing imaging.
The examination may include spinal and hip movement, gait, straight-leg or nerve-tension testing, strength, reflexes, and sensation. Computerized spinal assessment may provide additional information about spinal function, but it is interpreted with the history and physical findings.
X-rays may be requested when structural assessment is clinically relevant. MRI, medical testing, or specialist referral may be needed when symptoms are severe, progressive, atypical, or not explained by the examination.
